Keep clinical detail off this form. Enough to know whether we can help and how to reach you is plenty. Anything more belongs on the phone, with a release in place, and it is your client's information to give rather than ours to collect.

We will still need to speak with the person themselves before anything is arranged. Placement has to be their decision, and we cannot discuss their information with you without their written permission.

What speeds it up

  • A direct number for the person, not only for the file
  • Their discharge or release date, if there is one
  • Whether they are working or looking for work
  • What supports are already in place
  • Any supervision conditions attached to the placement

None of it is required. It just turns a week into a couple of days.

What we can document for you

With the resident's written permission, we can confirm residency, provide attendance and compliance letters for courts and supervising agencies, and let you know promptly if a placement ends.

Without that permission we cannot confirm anything, including whether they contacted us. That is not us being difficult; it is the resident's information to release.

No money changes hands

We do not pay for referrals and we do not accept payment for making them. If anyone in this industry offers you either, that is worth reporting rather than accepting.
